Thank you for your email.  Dr. Brown could see you tomorrow, Jan 23, 2013 at 3:30PM for a consultation regarding your tooth.  Is there a possibility that you download a New Patient form and have it filled out by the time you come in?
If not please do come in at 3:20PM.

Are there any x-rays of the tooth in question that you can bring in for tomorrow ?
Please call me as soon as possible to confirm this day and time.
